As a volunteer of MAKE 'm SMILE Ohio, I commit to fulfill the responsibilities appointed to me which include:

Thursday, 7/7 at 6 PM at Harvestime Apostolic Temple – 136 E Maple St., Hartville, OH 44632
Friday, 7/8 @ 6pm (Location TBD - church or park)

Arrive at the event at 9:00 am (or at time needed for set-up crew) and staying until 6:00 pm (or as late as needed for clean-up crew).

Following the guidelines given by my team leader.

Doing my best to portray quality character as a representative of Nathaniel's Hope.

As a volunteer for Make 'm Smile, I grant permission for Harvestime Apostolic Temple and Nathaniel's Hope to use any images or video shot by our photographers for promotional purposes.
